Can be accessed by defeating Ophanim and then using a Sigil of Eternal Light on the locked gate.

True Ophan, The Forsaken Observer

Boss Phases

100% to 85% (Phase 1) | True Ophan teleports to a random edge within the bounds of the arena and has two small rings and one bigger ring circling around him. True Ophan summons eyeballs from the middle of the two smaller rings, and arrows from the middle of the bigger ring. The eyeballs move faster, but deal less damage while the arrows are slower but deal more damage. After a couple seconds, True Ophan will teleport to another random edge and repeat the process.
Guaranteed at 85% (Phase 2: Walls)| True Ophan's eyeball will lie down in the middle of the arena and summon walls that enclose within the player. Within the walls is a randomized gap that the player will need to go through to avoid taking damage. When the walls enclose within the middle, particle will appear indicating where the gap will be when the walls spew out of True Ophan. This process will repeat until the phase is over or if the player can damage True Ophan fast enough to move onto the next phase.
85% to 60% (Phase 2: Eyeballs) | True Ophan will stay in the middle and summon two rings of arrows that the player will need to stay in between of. These rings will continuously increase and decrease in diameter meaning the player will need to stay between the continuously moving gap in order to survive. While this is occurring, eyeballs will travel towards True Ophan which the players will need to avoid as well. If the player does not phase True Ophan by the end of this attack, True Ophan will go back to doing Walls and repeat all of Phase 2.
60% to 40% (Phase 3: Clock) | Ophanim will teleport to the center of the map and transform into a midnight colored clock. He will then shoot eyes in a spiral, followed by a flurry of shots, followed by a second spiral. Upon the clock striking midnight, the shots will freeze before returning to Ophanim at high speed. This phase is identical to the one from Ophanim, except the projectiles move much faster.
40% to 15% (Phase 4: Wheel) | When True Ophan phases after Clock, the boss can randomly choose between two moves, one of which is Wheel. During this attack True Ophan will wheel across the arena and whenever he hits a wall a barrage of projectiles will be unleashed and lingering arrow projectiles. The barrage of projectiles is on a cooldown of 0.3 seconds after hitting a wall.
40% to 15% (Phase 4: America) | When True Ophan phases after Clock, the boss can randomly choose between two moves, one of which is America. At the start of this attack, an indicator will appear on a player, showing who True Ophan will teleport to. After teleporting, the boss unleashes a spiral of large projectiles in either a clockwise or counterclockwise direction, while a spiral of smaller projectiles simultaneously rotates the opposite way (e.g., if the large projectiles move clockwise, the smaller ones will move counterclockwise). This sequence repeats three times, with each teleport becoming progressively faster.
15% to 0% (Phase 5: Grid) | At the start of this phase, you have a few seconds to position yourself within a gap in the grid while avoiding the large rotating beams, the large stationary beams, and the small rotating beam. This pattern then repeats indefinitely until the boss is defeated. To avoid the larger beams, you just need to position yourself away from them. The small rotating beam can be jumped over. Do note that it is ill-advised to stay in the fight if you are about to get hit by a larger beam as they have the potential to kill you in under 5 ticks.
